All unique jewelry must be obtained by defeating the boss of difficulty 4, and the difficulty level depends on the operation of Atlas point system:

Unique Jewels are very useful, but they are also very difficult to obtain. Difficulty 4 pinnacle bosses are arguably the most difficult challenges in POE 2.
